PatientLink Scanning System
The PatientLink Scanning System uses Optical Mark Reader (OMR) technology to easily capture patient responses and send them into the electronic record. The data is then available for easy viewing within your chart update.The most popular scan forms are those which capture past medical history, family history, social history, risk factors, and review of systems. Some specialties also use PatientLink to gather the History of Present Illness.
Scanning Scored questionnaires such as PHQ-9, SF-12, SF-36, Oswestry, Beck Depression, and KC Cardiomyopathy is a breeze with PatientLink. The questionnaires are scored and the patient responses plus the scores are sent into the health record in seconds!

You may want to add PatientLink Online to capture data from patients who are willing to complete the questionnaires at home. PatientLink Online is offered as an add-on product for clients who are using the PatientLink Scanning System.We can build a web form which is identical to your PatientLink Scanning System form. This form can be placed on your secure website to allow patients to complete the information before coming to the office. The data will flow into your patient chart in the same format as if it were scanned in the office.
